Description Mackenzie Richmond Hill Hospital and future Mackenzie Vaughan Hospital two-hospital model of care plus Network of local and Barrie community-based services, plus Saint Elizabeth Rehabilitation Health Centre Partnership * Services include: Emergency * Acute Inpatient * Ambulatory * Complex Care * Rehabilitation * Diagnostic * Palliative * Interim Long-Term Care and Long-Term Care services * Mental Health Program * Continuing Care Program * Woman and Child Program * Childbirth Education Classes * Genetics Clinic * Family Birthing Centre * Child Day Care Centre for staff and public * Multi-faith Chaplain available Monday to Friday from 9 am to 3 pm * Rabbi available Monday to Thursday from 2 pm to 6 pm * contact either by asking your nurse or social worker to call, or contact the Chaplain at ext. 7562 or the Rabbi at ext. 7871

Saint Elizabeth Rehab Health (preferred rehab partner) Bayview Site * Outpatient Rehabilitation and Physiotherapy 
Vaughan Community-based Services include: 
Upper Thornhill Centre Site Health and Wellness Centre Site * Cardiac Rehab * Diabetes Education Centre * Domestic Abuse & Sexual Assault Care Centre (DASA) 
Jane Street Site * Chronic Kidney Disease Program  
Barrie Ontario Community-based Services include: 
Bryne Drive Site * York/Simcoe Centre for Behaviour Health Sciences Behavioural Clinic • Sexuality Clinic • Autism Services • York-Simcoe Brain Injury Services

Accessibility Fully Accessible ; Please ask a nurse or Patient Care Coordinator (PCC) to contact Communication Services for special phone services or to contact someone who can provide sign language services.
