The Central CCAC helps Ontario residents of all ages - from babies who are just a few days old to seniors of 100 years or more.  
 
You may be eligible for CCAC services if you:  
 
- are recovering at home after a hospital stay  
- have a disability or serious illness  
- need help with your personal care for medical reasons, or  
- need help caring for someone on an ongoing basis.  
 
You must have a valid Ontario health card,live in the Central CCAC catchment area and meet eligibility criteria to receive services funded by the Ministry of Health and Long Term Care. Anyone can call us for an assessment. If you are eligible, a Case Manager will help you and your family build a community health care team to meet your needs. Case Managers coordinate and authorize payment for client services.
