| Description | Community & Home Assistance to Seniors (CHATS) is dedicated to assisting seniors live independently and providing care and support to caregivers. Home Support Services include: Homehelp; Personal Care Services; Respite Care or Caregiver Relief. Subsidy available. Community Programs include: Meals on Wheels; Lunch Out; Wellness Programs; Adult Day Program; Caregiver Education; Diversity Outreach Programs; Lifeline Personal Response; Friendly Visits ; Transportation Services for medical appointments, Shopping Club, Telephone Reassurance, and special outings; Subsidy available. Home At Last, a free service that makes the transition from hospital-to-home easier and more pleasant. Patients are driven home and a Personal Support Worker helps make them comfortable back in their homes. Balance of Care (BOC) Program is a new and innovative model being piloted in the Richmond Hill and Downsview areas of the Central LHIN by delaying or preventing admission to Long Term Care by offering a basket of services to seniors including non-medical services (i.e. Meals on Wheels, Transportation, Adult Day Program) normally offered by community support agencies. MediChair-CHATS Home Medical Equipment Store in Newmarket and kiosk in Richmond Hill. Home Adaptation and Maintenance Program, offers seniors home adaptations such as ramps, handrails, grab bars, stair lifts, lighting and door knobs or maintenance such as yard cleanup, leaf raking, tree pruning, snow removal or house cleaning. This program is offered to senior residents of Georgina, East Gwillimbury and Bradford/West Gwillimbury townships. CHATS also offers Diversity Outreach Programs for isolated seniors to support wellness and healthy aging in under-served communities. Activities include: socialization and cultural celebrations, information on health and community services, nutritious and culturally appropriate snacks, low impact exercises i.e Yoga, and English conversation clubs. | |||
